Intership Forms

The Internship Forms

During the internship process, many forms need to be completed.  Here are the major forms that will be necessary to complete your registration for internship credit.

 

Syllabus for MMBIO 399R - This the offical class syllabus for academic internship credit. This will help you understand grading policies, what will be required of you, and the process of securing your internship.

 

The Student Agreement Form - This is the first form that you will be asked to fill out. It is the most important form for keeping track of you. Some of it you may not be able to fill out until you actually arrive at your internship, but we need as much contact information as possible before hand.  This form basically demonstrates that you, as the student, agree to be enrolled as an intern, represent BYU honorably, and accept the conditions listed at the bottom of the form.

 

The Master Agreement Form - This is a university form. If you are seeking credit, then your provider (the university, business, person, gnome, etc. that is offering the experience) must sign one of these. We will help you with this process. Basically it states that the provider agrees to follow all federal employment regulations based on your status and to complete any contract that they have with you, the student. Also it says that the student agrees to follow all normal university policies, including the honor code. In return BYU agrees to cover each student with a $3,000,000 insurance policy. PLEASE NOTE: The last two pages DO NOT need to be filled out by anyone. They are only there for reference. They constitute all of the information filled out on your Student Agreement Form, and are only there so that the person signing the form can see what you have agreed to and the information that we have available at their request

 

Final Interview Questionnaire - These are a base for the questions that will be asked in your final interview, after you complete your internship experience. You will probably be asked more questions, but they will not be anymore difficult than the ones that you see here.

 
Evaluation of Intern Form
- this will be sent to your supervisor, and will be the main basis for what your grade will be.
