Form A- Application
Form B- Honor Code Commitment & Confidential Report
Form C - Letters of Recommendation (3)
Form P- Program and Fee Payment of $50.00
In addition, the following is required:
STATEMENT OF INTENT
BACCALAUREATE DEGREE - in a science or chemistry related field
TRANSCRIPTS- Two official copies
GPA: 3.0 or greater on last 60 hours of course work
GRE (Graduate Record Exam): Official Report (1150 or above)
Institution code: 4019, no department code is needed.
INTERNATIONAL STUDENTS
- in addition to items listed above, the following items are required:
FORM I-1- Financial Certification Form (found in the Application Packet)
CREDENTIAL EVALUATION- For degrees awarded outside the USA, all official transcripts, diplomas, and marksheets must be submitted to one of the following agencies for evaluation:
Education Credential Evaluators (ECE)
International Education Research Foundation (IERF)
World Education Services (WES)
LANGUAGE TEST:
